Long COVID is one of the defining chronic health challenges to emerge from the pandemic. An estimated 10 to 20 million Americans, and more than 420 million people worldwide, live with symptoms that persist long after the acute infection resolves. There are no approved treatments. Patients report more than 200 distinct symptoms spanning as many as 12 organ systems, from cardiovascular and neurological to respiratory and gastrointestinal.
Mounting evidence indicates that SARS-CoV-2 does not always clear when symptoms of acute infection fade. In a subset of patients, the virus persists in tissue reservoirs for months, and in some cases years.
This persistence is increasingly understood as a driver of chronic disease. Fragments of viral RNA and protein have been detected in biopsies, blood and stool samples long after recovery, and these reservoirs are thought to sustain the chronic inflammation and immune dysregulation that characterize Long COVID. This review synthesizes that evidence and sets out the scientific rationale for a T-cell immunotherapy approach across four objectives.
One: Establishing that the virus persists
The first objective is to consolidate the direct evidence of persistent virus. Studies using biopsies, blood and stool samples have repeatedly identified viral material in patients months after their acute infection, supporting the hypothesis that a hidden reservoir, rather than a single past event, underlies ongoing symptoms.
Two: Mechanisms of chronic inflammation
The second objective examines how these reservoirs sustain disease. Persistent antigen appears to keep the immune system in a state of chronic activation, producing the inflammation and tissue dysfunction reported across organ systems. Understanding these mechanisms clarifies why symptom-focused treatments have fallen short.
Three: Animal models of persistence
The third objective is the development of animal models that reproduce viral persistence, providing a controlled system in which to test whether clearing the reservoir resolves disease. These models are essential to translate the hypothesis into a testable therapeutic strategy.
Four: T-cell immunotherapy strategies
The fourth objective lays out the case for T-cell immunotherapy. Because T cells are responsible for identifying and eliminating virally infected cells, restoring and directing protective T-cell responses offers a path to clear persistent reservoirs at their source.
